{"id":3329,"date":"2014-05-07T09:00:16","date_gmt":"2014-05-07T13:00:16","guid":{"rendered":"http:\/\/blogs.mml.org\/wp\/inside208\/?p=3329"},"modified":"2014-05-07T09:00:16","modified_gmt":"2014-05-07T13:00:16","slug":"evip-category-3-due-june-1-2014","status":"publish","type":"post","link":"https:\/\/mml.org\/inside208\/2014\/05\/07\/evip-category-3-due-june-1-2014\/","title":{"rendered":"EVIP Category 3 Due June 1, 2014"},"content":{"rendered":"<p>The EVIP\/CIP Category 3: Unfunded Accrued Liability Plan is due on June 1, 2014.<\/p>\n<p>Submissions can be emailed to <a href=\"mailto:TreasRevenueSharing@michigan.gov\">TreasRevenueSharing@michigan.gov<\/a> or faxed to 517-335-3298 up to (and including) 11:59pm on Sunday, June 1, 2014.\u00a0<strong><br \/>\n<\/strong><\/p>\n<p>Please check Treasury\u2019s <a href=\"http:\/\/www.michigan.gov\/treasury\/0,4679,7-121-1751_2197_58826-312694--,00.html\" target=\"_blank\" rel=\"noopener\">website<\/a> for additional information, including the required Certification Form 5074, available templates and FAQ\u2019s.\u00a0 The list of required documents is also available on Treasury&#8217;s website.<\/p>\n<p>When filling out the Certification Form, the \u201cContact Name\u201d should be the person who can answer questions pertaining to the submitted plan.\u00a0 Usually this is the person who has completed the plan.\u00a0 The Chief Administrative Officer\u2019s name does not need to be the contact name (unless he or she can answer questions pertaining to the submitted plan).<\/p>\n<p>Additionally the League has worked with MERS to put together a template that may be used when sending information to Treasury.
